Goals

By talking and planning together, we work with people using Pathways' services to identify and set goals - and plan a pathway for achieving them. Pathways support workers work with people to help plan the type of support they want to receive.

Support workers are part of a wider team which includes other community support workers, registered health professionals, healthy lifestyles coordinators, housing facilitators, team leaders, service leaders, administrators and regional managers.

Creating a plan

Once goals have been identified, Pathways support workers work with people to create a unique and personalised plan. This can be in any form; some people may want to create a journal, others work with mind maps, a dream book or an action plan.

People choose who they would like to be involved in the development of their plan. Family/whanau or friends may be invited to be involved, or specialists such as education or employment advisors.

Identifying opportunities

Over the years, Pathways has identified many ways to support people to achieve their goals. Working, learning, getting involved in activities and being part of the community are things we all value. Our support workers have a wealth of knowledge about what’s on in the community and how to become involved. We make it our business to keep up-to-date with services, events, programmes, courses, workshops and opportunities that may interest people. We also attend regular networking events to ensure that we have a wide range of contacts, resources and information.
