Goals

By talking and planning together Pathways works with people using our services to identify and achieve their goals.

One-to-one support

Pathways will talk to people to help plan the type of support they want to receive.  This is mostly done through a ‘key worker’, who is a skilled community support worker.

Key workers are part of a wider team which includes other community support workers, registered health professionals, healthy lifestyles coordinators, consumer advisors, housing facilitators, family/whanau advisors, service leaders, administrators and regional managers.

Creating a plan

Once goals have been identified Pathways will work with you to create a unique and personalised plan. This can be in any form; some people may want to create journal, others work with mind maps, a dream book or an action plan.

It’s the individual’s choice about who is involved in their development of a plan. Family/whanau, friends or a partner may be invited to be involved, or a specialist such education or employment advisors.

Identifying opportunities

Over the years, Pathways has identified many ways to support people to achieve their goals. Working, learning, getting involved in activities and being part of the community are things we all value. Our support workers have a wealth of knowledge about what’s on in the community and how to become involved. We make it our business to keep up-to-date with services, events, programmes, courses, workshops and opportunities that may interest people. We also attend regular networking events to ensure that we have a wide range of contacts, resources and information.
